Probably the first thing you need to do is find a diet that you can stick to, and an exercise routine, probably 30 min / 3 times a week would be a good start

Weigh yourself once a week. Keep a log of your weight-loss to stay motivated. You could also take measurments!
Set acheivable goals for yourself and give yourself rewards when you get there
