I'm working out almost every day (cardio and weights) and counting calories. I have a big chart on the wall in my kitchen where I track calories, workouts and weight. I'm only 4 days into it but I intend to stick with it this time. I know from experience that it's the long term commitment that makes a difference... a little at a time.

and for keeping almost all of it off. You might want to check out the Maintainers Forum (not just for people at goal) - we have a 'sticky' thread called Relapse that might be interesting to you. Many of us have found that maintenance is a lot like a roller coaster - up and down - and we have to work hard every day at keeping it heading in the right direction.
