The last time I seriously tried to lose weight (2 years ago), it took me about 35-40 pounds before I noticed a difference (and people I saw all the time noticed a difference too). I started at about 380 that time, and I started at 380 this time too, so I'm expecting it to take until I get down to 345 or 340 before I seriously notice a difference. (FTR, I'm 5'4")
I remember last time wearing a pair of pants and turtleneck (which I still own) that, when I was at 380, I was constantly pulling my pants up and my shirt down, because while they did fit, the way I'm shaped (I carry a large proportion of my weight in my tummy) and the fact that I have a really long torso and short legs meant that my pants would slide down and my shirt would ride up as I walked. I have a memory of my former boss commenting that my clothes were hanging off of me and I needed to get some new ones. Sadly, shortly after that, the HR person at my old work made a snarky comment that I needed to shut up about the weight loss because it was getting old, and that threw me completely off program and I'm just now finally getting back to getting serious about it.
