I was in that situation last year. Total burnout. Maybe take it one thing at a time? I went into a major funk and my grades suffered tremendously. Try not to put so many things on your plate, or at least re-arrange them

. If you can salvage the semester, then focus on that. With me, getting back on track (emotionally, physically, and academically) required some major decisions and me slowly piecing everything back together, taking away as many distractions I could find. I even quit my job! And I second talking to a counselor at school as well as an academic advisor. Just talking to someone who knows the ropes and can give you some options migh help take some of that heavy load off.
Also, you said you were so angry with yourself you were going to start getting up early to workout. Can advise to nix that? Putting a negative spin or adding the punishment factor to something that is supposed to make you feel better will only add to the stress if you don't happen to make it to the gym. For me that just digs the hole even deeper.
