I'll second the midwife vote. Part of the woman-centered care tends to be answering your questions.

Midwives also--usually--won't push you into unnecessary tests, and your danger of a c-section will be less with one as your attendant as well. I'll also put in my vote for homebirth, even with your first. Research shows homebirth to be just as safe as hospital birth, and I can say from experience (although I wound up transferring) that looking forward to birthing at home made this last pregnancy much calmer for me.
