I mentioned this in another thread, but setting small goals is a great way to get the ball rolling!
For example, set a goal of going to the gym 3 times THIS week. EASY! It's all about winning the small battles and gaining momentum. Soon it will be more natural to be goal oriented. Making lists helps because crossing things off feels oh so good.
