I can understand your frustration in not seeing the results you're looking for. It may seem like you're doing everything you can, but weight loss is about living smart, not hard.
You really have to keep track of what you're eating with a food log. This is the ONLY way you'll know what's going into your body, when, and in what amounts. You might be surprised once you write it down.
In regards to exercise, it is more important the intensity you are working out, not the frequency. Instead of just getting through that 45 minute cardio or weight session, concentrate on pushing your body to new limits during every workout. If your body isn't continually stressed beyond the point of comfort, it's not going to change its composition.
