When I lived in the Dallas/Fort Worth area of Texas, my husband and I would always get a sandwich with spinach.
Where we live now, none of them have spinach so this issue doesn't affect us (most unfortunately).
I don't put up with crappy service. I'll tip well if you do well (20%+), 10-15% if you don't screw anything up. But man, if you are rude, I will rip you a new one. One night, hubby and I were at a restaurant and got the worst service. We had to get up and ask the bartender for refills, carry out boxes, etc. Our waiter was too interested in the football game on tv to check on us. He got zero tip and a nice paragraph on the back of the receipt why so his boss would see it too.
Oh, and to the person who had their tip inflated - that's credit card fraud and I would have called the manager and asked for a copy of the receipt, disputed it with the bank and went to the cops. That's a stones throw away from stealing someones card information because you want to reward yourself with a new pair a shoes. [/end
