Kick Start Menu

Hi, does anyone have the kick start menu or know where i can find it here or online? I have it at home but wanted to start this morning at work and forgot it! I think i need this to get me going again! I'm in a lull of back and forth 1 lb.

I haven't weighed in since 2 weeks ago and feel i'm slipping out of my routine.
