Well you mentioned that you never keep track for more than a week. So if you are following a strict diet of 1200 calories, cereal diet, etc, the reason one might give up is because they are hungry/not full filled. The lesson here would be: eat more. See what I'm saying?
What I've done is I start with small changes, no more pop, wearing a pedometer, eating less seconds or incorporating more vegs.
