I think it depends on the type of person you are & what works best for you. Most people quit one thing at a time. I'm the type that always seems to march to a different drummer: I figured if I was going to be "miserable" why not be really "miserable" & do it all at once!
I did Medifast, quit carb overloading, sugars, caffeine & smoking all at once. As I remember, I was NOT so pleasant to be around the first couple of weeks!
I think poor DH was scared of me for a little while there.
It did get better & I got all that angst over with at once. 
You got some great pointers on your diet & weaning yourself off caffeine. Having an afternoon snack of a little protein does help with the before dinner hungries. And if it doesn't seem like it's working to quit so many things at once, just take them one at a time. The important thing is to work on your goals (whatever method you choose). and I've been trying to cut the coffee (hence sugar in it) and I have found myself shaking an starving around 5:00 pm which yesterday turned into a junk food disaster...
