Need Help Finding Something

I While Ago Someone Posted On Here A Tracker. It Is One That You Keep Track Of What You Eat, How Much Water, Ect. It Was A Pdf File And I Can't Fine It Anywhere. Does Anyone Have One Available Taht They Can Pm Me Or Even Post Here? I Have Looked All The Way Back Through Page 14 And Can't Find It. Please Help.
