I use The Daily Plate (which I think is now part of Livestrong.com). I use it for tracking my food (but not my exercise) and for tracking my weight, measurements, and BMI.
I'm a big fan of fitday. I use it to record my weight and exercise, but not my food . It will make a graph of your weight automatically. I hear that ticker factory also has a graph tool, you could check them out, as well. Or if you have Photoshop or even MS Paint you could always make your own. That's what I usually do if it's a graph I'm going to be posting somewhere.
I made a weight loss sticker chart. Each page has 20 squares (5 columns, 4 rows) and in each square is the number of pounds lost, and my weight at that loss (so the first box has 393 in the top corner, and 1 in the bottom).
When I lose the pound, I put a sticker in the appropriate box. If I regain I don't remove the sticker, but I can't get the next sticker until I've lost all I gained, plus another pound.
Each row is 5 lbs, and each page is 20. I write a little reward at the end of each row (something cheap, but specific like a particular paperback or magazine or particular MP3 - once I write it down, I can't buy it for myself until I lose the 5 lbs).
It's a silly game, but it helps me stay motivated and see my progress.
I use a beautiful spiral journal to keep track of my calories (and sometimes, fats, protein and carbs). I have a running total that has dates and weights that I staple in the back of each journal as I need to start a few one. I keep track of my progress.
Every day that I stay in my calorie range and put a smiley face on that day. When I accumulate ten - fifteen smileys - then I treat myself to a new lipstick, socks, etc.
Folks do it so many ways.
When I first started I kept track of my food at Sparkpeople. They have graphs and charts too.

I use a program currently called iscale which I use on my iphone. It does't have a place for excercise yet and it doesn't have a graph yet, but the developer is working on it big time so those features are coming and I have it with me all the time!
If you just want weight loss, use a spreadsheet. Date, weight, body fat percentage (if you have it) and then maybe a column for notes or related. It's easy, fast, and you don't have to create an account anywhere. And I don't know a spreadsheet program out there that can't generate pretty graphs lickity split.
As far as online goes, I've used FitDay and The Daily Plate, but both for food and neither for graphing. So ... no help there.
I use a desk planner. I write down all my food and calories and its nice to look back and see how much i lost when and the different things i ate. I also include water intake and exercise.
My absolute favorite is calorie count plus. it has a graph and a place to write myself a note. Also it shows how I'm progressing. I also enter my food into it and I found it has more of the foods I eat in it's database than fitday (ex. Boars Head deli chicken).
