OMG, Sara! What a nightmare!! I've had similar experiences, but not quite as mortifying. The admin at my work asked me what size of shirt to buy as a "thank you" for working on a particular project team, and I told her, but somehow she ordered one two sizes too small. I never thought of the fact that maybe those particular shirts weren't offered in my size.
The nice thing now is that I can fit into that shirt comfortably! I no longer work there so it's irrelevant to wear a shirt celebrating a different company's project success, but I kept the shirt and I'm glad I did, just for the principle of the whole thing.
