I use fitday.com. It is free, and I can access it from anywhere. It includes all of the functions you asked about...tracks weight, exercise, foods, and progress at specific activities. It also tracks specific nutrition goals (good if, say, you want to get X number of protein grams or hit the DV for calcium every day). I love it, and no matter where I am, if I have internet access, I can get to it.
