This is me, too. I don't have any problem falling asleep, but staying asleep is another issue entirely.
What I've done:
*wear a blindfold
*wear earplugs
*have soft, fluffy, warm blankets (I get really cold)
*no caffeine, ever, at all
*cut down on carbs in the afternoon/evening
*on nights I have to sleep, I take Simply Sleep (OTC)
I'd love nothing less than to just go to bed without having to worry about all my rituals being in place...but since I can't, I do the best I can!
