Default All ye Photoshop experts!

I need help. I want to make this my avatar picture but I can't seem to do it. When I get it down to the required size it's tiny and grainy and aaargh!

Please, help! (and tell me how you did it!)

Or if you have any suggestions, that would be great!

you can use photoshop's blur function (either the stylize layers or the actually blur brush) if it gets too spotty.
