You could try doing intervals on the treadmill, in other words add some light jogging and incline/decline work mixed to the walking. This should help get your fitness a little higher. I'd try different machines, too, like an elliptical trainer with arm handles. The more muscles you involve in the work, the better. Also try your routine outdoors, weather permitting. A little more weight work wouldn't hurt.

Should I be doing something different or is it just harder to take off weight that you've lost and regained?

I've heard that's true. But try more variety and little bursts of intensity in your work. Sometimes you get to "cruising speed" on your fitness plan. The body gets efficient at the work, so you're not burning as many calories as you once did. Cross-training and intervals can shake things up a bit. It's more interesting, too.
